Asheville Project –Controlling Your Health For Life: Patient Self-Management Program A diabetes management demonstration project that utilizes specially trained pharmacists to conduct face-to-face meetings with the employee (or eligible dependent) to help educate, motivate and empower each participant to better manage their condition. The program replicates a highly successful diabetes project developed in Asheville, NC. Employers waive (or reduce) co-pays for diabetic supplies and drugs. Employees voluntarily participate in regular visits with specially trained local pharmacists. The pharmacists are paid to conduct regular screenings, counseling and coordinate care with physicians Preferred pricing on prescription drugs, vision benefits, Preferred Provider Organization (PPO) and Open Access III networks. Click here for more information. Health Literacy – Ask Me 3 Ask Me 3 Educational Materials, Tool and Resources. Ask Me 3 is a program to promote awareness and solutions around the issue of low health literacy and its effect on health outcomes. It is designed to enhance communication between providers and patients. Value-based purchasing initiatives also known as Innovative Benefit Design Legislative and Public Policy Information Bridges to Excellence (BTE) This program was developed by employers, physicians, health care services researchers and other industry experts to create significant leaps in the quality of care by recognizing and rewarding health care providers who demonstrate that they have implemented comprehensive solutions in the management of patients and deliver safe, timely, effective efficient, equitable and patient-centered care. Healthy at Heart – Disease Management Program
It is a goal of the Tri-State Health Care Coalition to assist our members and their employees in maintaining optimum health. Healthy at Heart is sponsored by Pfizer Inc. and was initiated in 2005 to help patients manage cardiovascular disease. This free patient education program is available to coalition member employers and their employees with dyslipidemia (high cholesterol), Hypertension (high blood pressure) and Diabetes. These diseases involve the cardiovascular system and, if left untreated, can lead to stroke, coronary artery disease and even heart failure. A key strategy for addressing these risk factors is through education about the importance of prevention and management approaches. The Healthy At Heart™ program includes education and management tools for proper diet, exercise and lifestyle modification – an approach that is known for being most effective and efficient. Members enrolled in the Healthy At Heart™ program will receive twelve months of unbranded disease education in addition to useful advice on different health subjects. You will learn more about managing your risk factors for heart disease; which will assist you in meeting your health goals. You will also receive tips for living a heart-healthy life style. Program information is delivered through the mail and accessible through a special Web site. The Diabetes Control Network is a supplement to Healthy at Heart. If you qualify, you can choose to get 6 more mailings; which will have advice about diabetes and how to mange it. Enrollment forms are available to our members by contacting us at (217) 221-3460.
