Patient Self-Management Program (PSMP) A diabetes management demonstration project that utilizes specially trained pharmacists to conduct face-to-face meetings with the employee (or eligible dependent) to help educate, motivate and empower each participant to better manage their condition. The program replicates a highly successful diabetes project developed in Asheville, NC. Employers waive (or reduce) co-pays for diabetic supplies and drugs. Employees voluntarily participate in regular visits with specially trained local pharmacists. The pharmacists are paid to conduct regular screenings, counseling and coordinate care with physicians Preferred pricing on prescription drugs, vision benefits, dental benefits, EAP services, Dependent Eligibility Audits, Preferred Provider Organization (PPO) and Open Access III networks.
